Important info:

1. All vials will be ordered only after the consent form has been signed which can be found in Your portal.
2. If you are doing the payment plan you will also need to sign the payment agreement.
3. If you are not yet a patient at Biothrive you will be required to have an onboarding appointment and appointment with one of our physicians to get started. There is no fee for either of these appointments.
